Using Insurance For Sessions
Filing insurance REQUIRES a mental health diagnosis
Couple's sessions are not able to be filed by insurance
Often, insurance companies can provide more information about a specific plan to the
member (you) than to the provider (me.) Therefore, if you plan to utilize your insurance benefits, you are responsible for contacting your insurance provider directly before our first appointment in order to avoid any surprises when it’s time to pay your bill. I make no guarantees regarding reimbursement from your insurance, and you are ultimately responsible for payment in full. These questions can help you prevent surprises when it comes to billing.
Does my plan provide benefits for outpatient mental health services? Are these services covered by ____ (BCBS, United Health Care, etc.), or contracted out to another insurance company?
Is this provider (Christen Murphy, NPI # 1588106280) considered in-network for my specific plan?
Is there a limit on the number of visits I can have?
Is pre-authorization required for mental health office visits?
What is my copay and/or coinsurance? If a percentage, what figure is it a percentage of?
Do I have an outstanding deductible? If so, how much has been paid toward it and how much is left? Is there a separate deductible for mental health services?
What is my annual out-of-pocket maximum?
Ask for a call reference number and name of the representative.

Missed Appointment Policy
Regular attendance is crucial in ensuring effective treatment. Once your appointment is made, that time is reserved specifically for you.
If you have to miss an appointment, I kindly ask for 24-hours notice so that I may reschedule your time slot with another client. Missed appointments without prior notice will be charged full fee. This fee is not reimbursable by insurance and will be charged to your credit card on file.
Late cancellations due to inclement weather or sudden illness will not be charged. I am happy to work with you to reschedule your session time or offer you a secure and HIPPA compliant teletherapy session.
